Our case manager works with the shelters in the area to identify homeless individuals who are ready to be housed and works with those individuals to:
1.
Search for housing.
2.
Review their lease
3.
Assist them in the move-in process
4.
Subsidize their rent for a set period of time while they work to increase their income.
5.
Meet with them monthly to check on their progress toward housing independence.
SHELTERS IN THE ARE REFER CLIENTS TO THIS PROGRAM, SO AT THIS TIME THERE'S NO ONLINE APPLICATION.
